Abstract
The first exchange transfusion in pediatric department at the Medical Centre in Stip is applied on 07.01.1988 year. Blood was prepared by Dr. Nikola Kamcev specialist transfusiologist and administered by Dr. Stanko Kurciska, neonatal pediatrician in Skopje and Dr. Blagica Lozanova, neonatal pediatrician, in pediatric ward at the Medical Center in Stip. Over the next few years (until 1993) were made ten exchange transfusion.
The best way to treat newborns with severe hemolytic disease is exchange transfusion (ECT). Exchange transfusion is used in hemolytic disease of the newborns and hyperbilirubinemia when other methods of therapy (for example, the early and intense use of phototherapy) do not give satisfactory results.
Hemolytic disease of the newborn is disease that occurs as a result of incompatibility of the Rh factor in the blood of the parents or incompatibility of the ABO system of a pregnant woman and the fetus.
Exchange transfusion was conducted via umbilical vein. It was transfused fresh blood no older than 5 days of donation, with a hematocrit of 0.50 to 0.60. Red cells are taken from a donor with blood type 0, and plasma from AB Rh negative blood. Volume of 300-450 ml. If necessary, a few days after transfusion can be repeated.
