Abstract

Nanocomposite materials novelty in restorative dentistry and widespread due to their good characteristics. Nanocomposites meet expectations to cover the deficiencies of microcomposites and hybrids in terms of better aesthetics, but at the same time better mechanical resistance. Since the size of the nanoparticles is at a lower level than the visible light, it is impossible to absorb the light in them. This phenomenon is essential for obtaining excellent aesthetic features, especially in the frontal region of patients. The small particle size enables a greater amount of particles in the filler and good polishing. The smooth and polished surface of the restoration prevents the accumulation of dental plaque, therefore good oral hygiene can be maintained. The low value of the polymerization contraction of nanocomposites is due to the strong interactions between organic matrix and inorganic nanoparticles. This is of particular importance for the longevity of nanocomposite restoration and good marginal fit.
The aim of this paper was to evaluate which nanocomposite materials are used in dental clinics in Macedonia, Bulgaria and Sweden; whether a bonding material from the same composite manufacturer is used; the effectiveness of the nanocomposites in relation to the marginal fit, as well as the representation of the restoration model according to the anatomic-morphological characteristics. The research was conducted in 10 private dental offices in Bitola, Macedonia, 3 private dental clinics in Plovdiv, Bulgaria, and in a public dental clinic in Malmö, Sweden. The research sample comprised a total of 26 dentists, including both gender: 14 male and 12 female therapists. For the realization of this study, a questionnaire was prepared for the doctors and each of the respondents expressed their personal experience for the nanocomposite materials.
From the results we concluded that nanocomposite materials are often used in dental offices in Bitola, Plovdiv and Malmo for obturation in restorative dentistry. The most commonly used nanocomposite materials in the dental offices in Bitola are Filtek Ultimate (3M ESPE), Tetric EvoCeram (Ivoclar Vivadent) and Gradia direct (GC). The most commonly used nanocomposite materials in dental offices in Plovdiv are: Gradia Direct (GC), Synergy D6 (Coltene) and Filtek Ultimate (3M ESPE). According to the obtained analyzes, we can conclude that the most commonly used nanocomposite materials in the Folktandvården Skåne dental clinic in Malmö are: Filtek Supreme (3M ESPE) and Tetric EvoCeram (Ivoclar Vivadent). A greater percentage of the therapists in the dental offices in Bitola and Plovdiv use a different bonding material from the nanocomposite manufacturer, while the therapists of the dental clinic in Malmö use the adhesive system prescribed by the manufacturer of the nanocomposite. As reported by the achieved results from the examined dental offices in Bitola, Plovdiv and Malme, we can conclude that all therapists are satisfied with the use of nanocomposite materials and have achieved excellent clinical effects in relation to the marginal fit. A large percentage of the investigated therapists in this study perform a modelation of restoration according to anatomic-morphological characteristics, which has a significant impact on its longevity as well as good aesthetics.
