Abstract

Left atrial masses are rare but clinically significant findings, which can present as diverse pathological entities, including primary tumors, thrombi, and metastases. Their diverse pathological entities contribute to a wide range of clinical manifestations, often presenting with nonspecific symptoms that pose challenges for early diagnosis. The case is about a 52-year-old female with symptoms of persistent fatigue, dyspnea and cough in the last 9 months. Past medical history is negative except for hypertension which is treated with tabl. Lisinopril 10 mg 1x1. Pre admission work-up was impeccable except for lab values of haemoglobin of 103 g/L, the rest was normal. Contrast CT: The heart is of normal size without pericardial effusion, but there is a large, clearly limited and hypodense defect in opacification of the left atrium, 4x4 cm in the axial plane. Cardiologic exam on TTE noted left atrial mass that takes up ¾ of the LA. Patient was admitted for cardiosurgery treatment because In the left atrium there is a spherical mass with an area of ​​13 cm2, attached to the basal third of the interatrial septum, which fills 3/4 of the left atrium, is mobile and prolapses through the mitral valve in diastole and compromises the filling of the left ventricle, which is an indication for operation. The anesthetic approach included induction with Midazolam, Fentanyl, Ketamine, Etomidate and Rocuronium, afterwads 7.5 ETT was placed. Maintenance of anesthesia included the use of inhalational anesthetic – Sevoflurane, opioide analgesic – Fentanyl and muscle relaxant - Rocuronium, Propofol – for sedation during CPB. Postoperatively, patient was transferred to ICU where she remained for 5 days. Histopathological examination and Immunohistochemistry proved cardiac myxoma.
