Abstract

Acute coronary syndrome represents a set of three clinical entities: unstable angina, myocardial infarction with and without ST segment elevation. Differentiation between these three entities is done using serum levels of cardiac biomarkers, changes in the electrocardiogram, and the clinical picture. Chest pain is a major symptom. Acute coronary syndrome is responsible for number of deaths in the world and often represents recurrent condition with large impact on individual and collective health. Men are more commonly affected, while women predominate in certain age groups. The vast majority of cases of acute coronary syndrome are due to rupture of an atherosclerotic plaque. Troponin is the gold standard in the diagnosis of acute coronary syndrome, but its role in determining the risk of future events, specifically adverse cardiac events, is increasingly being explored. Tumor necrosis factor alpha is a cytokine that has different effects on different cell types. Its influence on the occurrence of atherosclerosis has recently been proven, as well as the future occurrence of adverse cardiac events after an episode of acute coronary syndrome. Major adverse cardiac events are a common occurrence in patients with acute coronary syndrome, and the use of biochemical variables in determining the risk of their occurrence is of great importance for their prevention
