Abstract

Introduction: Patients’ expectations are always high and compromises with implant-supported dentures are, therefore, particularly in the presence of a high smile-line, almost impossible. Implants performed without optimal preoperative planning and treatment can result in faulty implant positioning and/or insufficient management of the surrounding soft tissues which will result in an aesthetic failure.
Case presentation: This is a case report of a 27- year-old patient, after receiving a transmucosally placed implant in the region of fractured tooth #21.
Clinical and x-ray evaluation revealed adequate alveolar bone, absence of periapical pathology but a present fracture line below the crest of alveolar bone, limited to the tooth. After extraction, an endosseous implant was immediately inserted and provisional restoration was placed to avail the benefits like preservation of bone and tissue emergence profile.
Conclusion: Respecting all surgical protocols and parameters and subcrestal positioning of the dental implant, as well as minimizing the tissue trauma during flapless implant insertion, ensure stability and satisfactory aesthetic results.
