Abstract

Introduction: A ledge is an iatrogenically created irregularity in the root canal. It’s caused by overzealous instrumentation with canal walls, after a canal curvature. How the greater canal curvature is, the
greater risk of ledge formation is. This may be occur in glade path formation with an aggressive instrumentation using larger rigid instruments. Canal blockage by dental tissue debris can lead to aggressive instrumentation. The next step in additional instrumentation is iatrogenically formed perforation in the root canal. When there is a doubt for a canal ledge formation, great care must be taken to plan the access cavity.
Aim: The purpose of this study is to evaluate the diagnostic procedure and prevention of ledge formation.
Materials and methods: A review study is made to reach the purpose of our study, by using electronic research with PubMed, Web of Science and Embase Ovid. Keywords are entered in the field for research. Inclusion and exclusion criteria were used: restriction about language (only those articles that are written on English) and publication date (articles that aren’t older than 2010 year).
Results: Reviewing the literature we identified total 92 studies: 22 duplicate studies, that we excluded and 70 studies-the final number for reviewing. 87% of the reviewed studies shown that cbCT with different side positions is the most common diagnostic procedure. The most used prevention method in the literature (68%) is to use appropriate glide path files, especially manual files (45%) and less reciprocating and rotary instruments.
Conclusions: There is a need for careful treatment planning canal, which is fundamental for avoiding the ledge. We need to plan the canal preparation especially to reach the canal curvature. Also, irrigation with EDTA and careful use of pre-curved hand files with ISO 08 /10, to reach the working length is essential.
